This chapter reviews the existing information regarding the advances made in developing fungal vaccines. In general, infections with , , , and occur in otherwise normal hosts. There are at least two principal reasons why a fungal vaccine should engage innate immunity. The first is that optimal activation of the innate system enhances the likelihood that a vigorous cellular immune response will develop. The second reason centers on the fact that many of the patients who develop serious fungal infections manifest some form of immune deficiency. The chapter also concentrates on fungal vaccine development for , which is the most common species currently causing invasive disease. Important risk factors for candidal disease include impaired cellular immunity, intravenous catheters, exposure to broad-spectrum antibiotics, dysfunctional or absent phagocytes, total parenteral nutrition, and implantation of prosthetics. The chapter provides an overview of the accomplishments in the field of fungal vaccination. Clearly, there are numerous candidate vaccines and at least two have entered the early phases of clinical study. Although the usefulness of the monoclonal antibody (MAb) to polysaccharide or the antibody to hsp90 of is not known, the mere fact that these compounds made it into the clinical arena is a sufficient impetus for further development. The area is an exciting one, and one that requires an interdisciplinary approach utilizing expertise in fungal biology, pathogenesis, and immunology.
